OBJECTIVES/GOALS: This study examined patient perceptions on the benefits, barriers, and facilitators of conventional and complementary/behavioral pain strategies that can be offered in the ED setting including physical therapy, mindfulness, acupuncture, and yoga. METHODS/STUDY POPULATION: We conducted and recorded semi-structured interviews with 30 patients who presented to the ED with musculoskeletal pain. Interviews focused on patients’ perceptions of NP pain treatments, barriers/facilitators to utilization, and recommendations that would promote engagement. A hierarchical coding system was developed and refined using the interview guide, the Theory of Planned Behavior, and preliminary review of the transcripts. The iterative process of developing the coding system allowed us to identify preliminary themes. RESULTS/ANTICIPATED RESULTS: Patients believe education on pain and the mind-body connection would give a sense of pain control. Likely barriers to engaging at the ED include lights, noise, interruptions, and uncertainty of their medical status. Post-discharge NP treatment barriers are financial and logistical. Engagement can be facilitated by a desire to avoid opioids, familiarity with meditation practices, and consistent positive communication with the health care team. Patients desire evidence on effectiveness, including testimonials, and suggested NP techniques should be introduced early, with written materials used for post-discharge referral. Patients prefer in-person treatment but would appreciate a virtual option. The initial session should occur in-person to build trust and facilitate virtual session engagement. DISCUSSION/SIGNIFICANCE: Patients are willing to engage in nonpharmacological pain treatment, however the unpredictable ED environment, uncertainty of their medical status, and financial and time constraints are significant barriers.

Fibricola and Neodiplostomum are diplostomid genera with very similar morphology that are currently separated based on their definitive hosts. Fibricola spp. are normally found in mammals, while Neodiplostomum spp. typically parasitize birds. Previously, no DNA sequence data was available for any member of Fibricola. We generated nuclear ribosomal and mtDNA sequences of Fibricola cratera (type-species), Fibricola lucidum and 6 species of Neodiplostomum. DNA sequences were used to examine phylogenetic interrelationships among Fibricola and Neodiplostomum and re-evaluate their systematics. Molecular phylogenies and morphological study suggest that Fibricola should be considered a junior synonym of Neodiplostomum. Therefore, we synonymize the two genera and transfer all members of Fibricola into Neodiplostomum. Specimens morphologically identified as Neodiplostomum cratera belonged to 3 distinct phylogenetic clades based on mitochondrial data. One of those clades also included sequences of specimens identified morphologically as Neodiplostomum lucidum. Further study is necessary to resolve the situation regarding the morphology of N. cratera. Our results demonstrated that some DNA sequences of N. americanum available in GenBank originate from misidentified Neodiplostomum banghami. Molecular phylogentic data revealed at least 2 independent host-switching events between avian and mammalian hosts in the evolutionary history of Neodiplostomum; however, the directionality of these host-switching events remains unclear.

The mid-to-late Pleistocene Devils Hole δ18O record has been extended from 60,000 to 4500 yr ago. The new δ18O time series, in conjunction with the one previously published, is shown to be a proxy of Pacific Ocean sea surface temperature (SST) off the coast of California. During marine oxygen isotope stages (MIS) 2 and 6, the Devil Hole and SST time series exhibit a steady warming that began 5000 to > 10,000 yr prior to the last and penultimate deglaciations. Several possible proximate causes for this early warming are evaluated. The magnitude of the peak δ18O or SST during the last interglacial (LIG) is significantly greater (1 per mill and 2 to 3°C, respectively) than the peak value of these parameters for the Holocene; in contrast, benthic δ18O records of ice volume show only a few tenths per mill difference in the peak value for these interglacials. Statistical analysis provides an estimate of the large shared information (variation) between the Devils Hole and Eastern Pacific SST time series from ∼ 41 to ∼ 2°N and enforces the concept of a common forcing among all of these records. The extended Devils Hole record adds to evidence of the importance of uplands bordering the eastern Pacific as a source of archives for reconstructing Pacific climate variability.

Reanalysis and additional dating of the Devils Hole δ18O paleotemperature record confirm that the last interglaciation in the Great Basin (the continental analog of marine isotopic substage 5e) lasted ∼22,000 yr, consistent with the Vostok paleotemperature record which suggests a duration of ∼19,000 yr for this event in Antarctica. The three preceding interglaciations in the Devils Hole record (analogs of marine isotopic substages 7e, 9c, and 11c) range from 20,000 to 26,000 yr in duration. A ∼20,000-yr duration for the last interglaciation is consistent with TIMS uranium-series dated sea-level high stands. Thus, the widely held view that interglaciations were of 11,000- to 13,000-yr duration and constituted only about 10% of mid-to-late Pleistocene climatic cycles needs reexamination. The warmest portion of each interglaciation in the Devils Hole time series is marked by a δ18O plateau, signifying apparent climatic stability for periods of 10,000- to 15,000-yr duration.

The furnished barrow burials of Wessex represent a maturation of the Beaker rite during the Early Bronze Age in Britain. Many of these burials were unearthed centuries ago, when archaeology was at its most eager and insouciant, but – happily for us – there were often a few careful recorders on hand. Thanks to their records, the modern scientists engaged in the Beaker People Project can still follow the trail back to a museum specimen and obtain high precision dates – as in the case of the ‘Wessex 1’ grave from West Overton in Wessex reported here.

Arstract:
W(15 Å)/C(15 Å), W(50 Å)/C(50 Å) and W(100 Å)/C(100 Å) periodic multilaver films were prepared by magnetron sputtering and suhsequently annealed at 980, 730 and 500°C. The changes of layered and crystal structures were studied by x-ray diffraction. The results depended largely on the thickness of W layers and their initial crystal struictures of W. The kinds of transformations are classified into two types: one is the non-reacting type in the case of multilayers having the thinner W layers like W(15 Å)/C(15 Å), in which the crystallization of W layers prevails over the reactions between W and C layers; the other, the reacting type in the multilayers having thicker W layers like W(50 Å)/C(50 Å) and W(100 Å)/C(100 Å), in which reactions Prevail over crystallization. A tentative model in which the effective reaction rate between amorphous–W and amornhous–C is much smaller than that of crystalline-W(beta phase) and amorphous-C is proposed to explain the origin of the phenomena.
